Attention Deficit Hyperactivity Disorder (ADHD) and Autism Spectrum Disorder (ASD)
Adult Referrals
At the present time, the NHS is unable to accept referrals for diagnosis & assessment of ADHD or ASD in adults (over 18's) except for those with complex needs; examples of this would include those already under the mental health services, or those with learning disabilities.
However, the 'Right to Choose' pathway exists, which allows you to choose a non-NHS provider that best suits your circumstances. These providers work with the NHS to allow access to diagnostic services during this time. Alternatively, you can opt to pay privately for a service provider of your choosing.
Each provider has their own referral pathway, details of which can be located on the provider's websites (linked below) - please ensure to look at all available options and make the right choice for you. In most cases, you will be asked to complete a comprehensive questionnaire prior to booking an appointment with a GP to finalise the referral. Please ensure to bring the completed forms with you to the appointment.

Child Referrals
Child referrals for ADHD and ASD are being accepted by the NHS, but there are currently long waiting times for these services.
In order for a referral to be accepted, there is a comprehensive referral form that must be completed in full prior to the referral being submitted. This form includes sections that need to be completed by parents/guardians with the child and also the child's nursery or school. Most education settings have a special educational needs and disabilities coordinator (SENDCO) who can support you with filling in the forms.
Please download and fill in the relevant form below. Once this is complete, please book a GP appointment to discuss the referral. Please ensure to bring the completed forms with you to the appointment.
