Coronary Heart Disease
What is CHD?
Coronary heart disease (CHD) is a major cause of death in the UK and worldwide. CHD is sometimes called ischaemic heart disease or coronary artery disease.
The main symptoms of CHD are:
- chest pain (angina)
- shortness of breath
- pain throughout the body
- feeling faint
- feeling sick (nausea)
But not everyone has the same symptoms and some people may not have any before CHD is diagnosed
Coronary heart disease is the term that describes what happens when your heart's blood supply is blocked or interrupted by a build-up of fatty substances in the coronary arteries.
Over time, the walls of your arteries can become furred up with fatty deposits. This process is known as atherosclerosis and the fatty deposits are called atheroma.
Atherosclerosis can be caused by lifestyle factors, such as smoking and regularly drinking excessive amounts of alcohol.
You're also more at risk of getting atherosclerosis if you have conditions like high cholesterol, high blood pressure or diabetes.
CHD Reviews
Each year your CHD should be reviewed to check for progression or improvement in condition. It also allows for us to ensure that you are receiving the correct treatment and that you are up to date with current support and advice.
Unfortunately, the COVID-19 pandemic has caused a delay in getting reviews done, so we are introducing new ways of working to maximise convenience for our patients and efficiency for our clinicians to ensure we can get back on track as quickly as possible.
This service is for routine reviews, if you are unwell or are struggling with any symptoms, please call the surgery on 01438 715044 to book a same day appointment or call NHS 111. If it is an emergency call 999 or attend A&E.
There are two stages to completing your CHD review...
1) Attend for a blood test - when you are called for your review or if you have been sent a link to this page - a online pathology request will have been made for you. Please attend either QEII, Lister or Hertford County Hospital to have the blood sample taken - please note that this is now by appointment only (https://www.enherts-tr.nhs.uk/changes-to-our-walk-in-blood-test-service/) or you can book to come to the surgery. It is a non-fasting blood test.
2) Complete the 'CHD Review' questionnaire - please log in to SystmOnline to complete - click here to log in. You can request log in details at reception or via the 'Contact Us' section of our website. Alternatively, you can download and print the form below or collect a paper copy from reception.
Once your completed form and blood results are received, it will be reviewed by a clinician to see if any further action is required. You may then be contacted to discuss the matter further.
